Previous Experience working in primary care medical settings preferred.Licensure / Certification Required: Candidates must be licensed for clinical practice in the State of Oregon (Oregon Board of Psychology or Oregon Board of Counselors) or have the necessary clinical training to acquire independent licensure in the State of Oregon.Under The Direction Of: Evergreen Family Medicine DirectorPURPOSE OF POSITION: Responsible for implementing the behavioral health strategy at Evergreen Family Medicine. This strategy includes integrating behavioral health services with the medical departments and clinics throughout the organization, managing resources to meet the behavioral health needs for Family Medicine patients, and ensuring viable partnerships for patients who require behavioral or social services outside of the practice. KEY EXPERIENCE: The Director of Behavioral Health understands the Primary Care Behavioral (PCBH) model of care and brief, evidence-based interventions that support its clinical efficacy (i.e., ACT, CBT, etc.). As a key member of the health center, the Director of Behavioral Health supports the overall organizational goals and participates in addressing health community-wide issues. ESSENTIAL RESPONSIBILITIES: Provides brief assessment, intervention, and onsite consultation to the primary care team regarding psychiatric/psychosocial concerns. The position will require 50% administrative/programmatic oversight and 50% provision of clinical care services, which may adjust to 80% administrative/ programmatic oversight and 20% clinical care services as the program grows. The Director of Behavioral Health will be tasked with building a sustainable program with an engaged team of clinicians. Key responsibilities for the Director of Behavioral Health include recruitment, clinical training, workflow development, and building trusted relationships with the medical team. Setting: Evergreen Family Medicine (EFM) is a primary care medical group serving Douglas County centered in Roseburg, Oregon. EFM has 40 providers spread among 4 clinic locations. EFM also provides telehealth into Douglas County Schools. Providers include Family Medicine, Internal Medicine, Pediatrics, Ob GYN, Psychiatry, Nurse Practitioners, Midwives, and Physician Assistants. EFM intends to institute an integrated behavior health program as part of their Primary Care Patient Centered Home mission to bring accessible mental health care to our patients. Compensation: Salary range $120,000 - $150,000. Incentive-based bonus up to 10% of base salary. Additional benefits include sign on bonus, relocation expense, health insurance for entire family, CME allowance of $5,000 per year, generous PTO, and Pension Plan. Key Functions• Provides clinical oversight of behavioral health services for department staff• Helps create an environment for the department that promotes job satisfaction, team collaboration, positive morale, retention, and staff participation• As needed and in collaboration with the COO, seeks funding opportunities and prepares grant proposals and reports to funding sources as needed• Participates in understanding and responding to community mental health needs• Provides or oversees direct clinical supervision in one-on-one or group settings• Develops and maintains systems to improve patient access to services, patient flow and urgent needs, and time-effective treatment• Provides training or oversees training in current protocols, diagnoses, and clinical intervention strategies• Ensure compliance with standards related to clinical quality, documentation, and billing• Provides on-site behavioral health services in primary care clinic (and virtual services as needed)• Maintains a strategy that allows for timely access of BHC to EFM primary care providers• Responsive to provider requests for warm hand-offs, consultation, and other inquiries• Provides for initial assessment and/or behavioral intervention with patients referred by primary care provider(s)• Adheres to the principles of Acceptance and Commitment Therapy (ACT), cognitive behavioral therapy and other evidence-based approaches and best practices• Directs interventions aimed at helping patients develop self-management skills• Provides feedback, verbal and/or written to primary care providers• Provides brief, episodic follow-up interventions as needed• Collects and utilizes data from screening tools for patient care• Provides crisis intervention and evaluation services as needed• Dedicates time outside of the clinic to build strong working relationships with community-based resources• Maintains current community resource list and facilitates referrals as appropriate• Assesses care needs of clinics’ population and works with providers, Medical Director and Clinic Administration to develop relevant program services (including involvement with chronic disease self-management)• Understands the economic sustainability of the program, and develops strategies to show return on Investment to operations leadership and payers.Goals:1. Establish an effective and responsive Primary Care Behavioral Health program which is integrated throughout the primary care clinic system.2. Establish treatment modalities to ensure a population approach to behavioral care which prioritizes access, collaboration, and team-based care3. Hire additional Behavioral Health Consultants (BHCs) to meet program needs in coordination with the administrative team.4. Establish excellent working relationship with other EFM clinical teams.Knowledge/Skills/Abilities Required: • Knowledge of the Primary Care Behavioral Health (PCBH) model of care and the Patient Centered Primary Care Home (PCPCH) • Basic knowledge of medical disease states and treatments preferred. • Must be able to collaborate with medical professionals and can adapt to a fast-paced primary care environment. • Knowledge of Oregon Revised Statutes and Oregon Administrative Rules related to the provision of mental health services. WORKING CONDITIONS AND PHYSICAL REQUIREMENTS:1. Must attend provider meetings and other collaborative meetings, such as quality assurance and Case Management.2. Physical requirements for this position include: hearing, seeing, speaking, feeling, talking, stooping, kneeling, crouching, reaching, grasping, and repetitive motions.3. This position requires sitting for long periods of time and walking and/or standing is only required occasionally.4. This position requires exerting up to 20 pounds of force and/or 10 pounds frequently to move objects.
