    Job Summary

    Provides general medical care and treatment to individual(s) served in medical facility, such as clinic, health center, or public health agency, under direction of physician. Performs physical examinations and preventive health measures within prescribed guidelines and instructions of physician.

    Essential Job Functions

    Order, interpret, and evaluate diagnostic tests to identify and assess patient's clinical problems and health care needs.

    Record physical findings, and formulates plan and prognosis, based on patient's condition.

    Discuss cases with physician and other health professionals to prepare comprehensive patient care plan.

    Submit health care plan and goals of individual individual(s) served for periodic review and evaluation by physician.

    Prescribe or recommend drugs or other forms of treatment such as physical therapy, inhalation therapy, or related therapeutic procedures.

    May refer individual(s) served to physician for consultation or to specialized health resources for treatment.

    Maintain current knowledge of each individual’s medical condition.

    Participate in Interdisciplinary Team meetings as required.

    Serve on committees as required

    Regular and punctual attendance

    Keep staff well informed regarding health-related problems of the persons served; discussing reasons for treatments, medications, etc. with the appropriate parties

    Provide in-service training to staff on medically-related issues

    Ensure that all policies, procedures, regulations, and guidelines are followed per state and agency requirements

    On-call availability for medical concerns

    Qualifications/Experience/Job Knowledge

    Graduate from an accredited school of Nursing. Must possess and maintain current registration.

    Master’s degree from an accredited advanced Nurse Practitioner program.

    Successfully pass the Certification Exam Experience

    Minimum of one (1) year clinically relevant experience is required. Experience working with elderly, or intellectually disabled preferred.

    Must have knowledge of and technical skills of medical nursing procedures, first aid, and basic principles of Nursing. Must know or learn about individuals with intellectual and developmental disabilities (IDD), seizure disorders, Autism, and other associated diagnoses pertaining to the field of intellectual disabilities and geriatrics.

    Working knowledge of Windows 7, Office 2010 applications. Aptitude to learn other software programs as required for this position.

    Must have the ability to maintain good working relationships with Evergreen staff and with contacts from other agencies or entities

    Must be able to prioritize work tasks

    Must be able to work without close personal supervision

    Supervisory Responsibilities

    Will not supervise.

    Employment Variables

    Must have a good driving record that meets Evergreen's minimum requirements and reliable transportation. Must pass a drug screen and criminal background check. Must be able to work some evenings when required.
