    Job Description

    DESCRIPTION

    Admission Care Coordinators are responsible, without little or no supervision, for screening and qualifying each new lead, providing necessary information to prospective client’s families and/or referring professionals who are seeking treatment for adolescents, and completing a thorough and comprehensive clinical assessment. Admission CM should be well versed in the admission process, have the ability to manage all aspects of an admission, coordinate with appropriate team members, and ensure for complete documentation in all required databases throughout the admission process.

    ESSENTIAL FUNCTIONS/RESPONSIBILiTIES FOR THE ADMISSION COORDINATOR:

    • Manages the complete admission process either from an initial call or a handoff from an opener.
    • Must be able to “convert” the call into a successful admission. This process may take 24 hours, or up to 6 months. Must maintain communication with family for as long as the conversion takes.
    • Must build rapport with the family from any stage of the process.
    • If a client is deemed not an appropriate fit, provide a clear next step/referrals to family’s based on the assessment of the case and knowledge of available services for teens.
    • Provide potential client’s families or referring professionals with information on the Evolve program and various levels of care in an in depth and educated manner.
    • Effectively explain criteria for admissions to family: both in regards to clinical and insurance criteria.
    • Screens each client to determine if they meet criteria and are clinically appropriate for the program.
    • Clearly and directly explain insurance coverage and payment options to families and/or providers. Negotiate payment arrangements if needed.
    • Maintain communication with family’s, sometimes after typical business hours and weekends.
    • Establish and report on new referents, clinicians, hospital systems to ensure for ongoing relationships. Communicate with marketing department on potential new leads.
    • Complete detailed Pre-admission assessment and gather all relevant clinical documents.
    • Use sound judgment when collaborating with previous providers gathering all pertinent records related to the case that will be important in assessing appropriateness of the client. This includes but is not limited to schools, therapists, psychiatrists, MD’s, hospitals, other treatment centers, educational consultants.
    • Gather necessary admissions documents/paperwork: including all consents and ensuring that all required documentation is collected in a timely manner.
    • Collaborate with internal stakeholders regarding the status of each admission to include Program Directors, Utilization Review Team and other appropriate staff; communicating and arranging physical, social, emotional, and support service requirements during the admission process.
    • Protects organization’s reputation by keeping information confidential.
    • Updates job knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks, attending networking events

    REQUIREMENTS

    • Ability to explain types of treatment modalities and terms used in a treatment setting for substance abuse and mental health treatment, (DBT, CBT, 12-Steps, Smart Recovery, group process etc.).
    • Familiarity with DSM-IV-TR and mental health diagnoses preferred
    • Work experience in mental health facility.
    • Previous experience in Admissions role is a must.
    • BA/BS or equivalent work experience preferred but not required with adequate industry experience.
    • Ability to be empathetic and an active, attentive listener.
    • The ability to work as a cohesive team member, to manage calls, documentation and communicate the status of cases in order to assist families and clients.
    • Clear and concise telephone and written skills.
    • Strong organizational skills: ability to keep information on each case separate and accurate and work on multiple cases at one time.
    • Must maintain confidential information
    • Must have the ability to work independently
    • Ability to effectively utilize computers, various software programs and cell phone technologies
    • Ability to multi-task and work within a fast-paced environment.
    • Two years of experience in a clinical or admissions setting is preferred.
