What You’ll Do
The Caregiver Field Support Specialist plays a key role in maintaining caregiver readiness and quality care delivery. You’ll be in the field supporting caregivers and clients, conducting quality checks, training visits, and coverage support as needed. When not in the field, you’ll serve as a flexible team member assisting with recruitment and onboarding efforts, particularly when our operations team is off-site.
Coordinate holistic care: collaborate with physicians, families, and caregivers to craft individualized Care Plans that meet medical, emotional, and social needs.
Ensure plan adherence: monitor daily services, review progress toward goals, and retrain staff when updates are needed.
Recruit & train caregivers: assist HR in hiring, onboarding, and educating field staff on best practices and agency protocols.
Schedule like a pro: build and adjust caregiver schedules for a caseload of ~30–40 clients, balancing continuity of care and employee preferences.
Track outcomes: document assessments, incidents, and goal progress in our EMR/CRM; generate reports that drive quality improvement.
Problem‑solve fast: resolve urgent issues with calm, confidence, and compassion—24/7 on‑call rotation shared with team.
:Field Support & Supervision
Conduct caregiver check-ins and support visits to monitor care quality and plan adherence
Provide hands-on training and skills refreshers to field staff
Support client onboarding by launching cases and introducing caregivers
Step in for supervisor visits, quality assurance, or urgent coverage as needed
Caregiver Readiness & Backup Staffing:
Assist with caregiver onboarding and documentation review
Help with recruitment efforts during job fairs, school visits, or other outreach events
Track and flag caregiver performance or retraining needs to supervisor
Maintain availability to assist with urgent scheduling or call-out situations
What You’ll Bring
Certification / license: CMA, CNA, LPN, RN, or Social Worker credentials welcome (preferred but not required)
1–2 years’ experience as a Care Coordinator, Client Care Manager, Case Manager, or similar role—home‑care or healthcare setting highly valued
Strong assessment, care‑planning, and documentation skills; familiarity with HIPAA and state regulations
Proficiency with Microsoft Office and EMR/CRM scheduling software
Excellent verbal and written communication; you build trust quickly with clients and clinicians alike
Stellar organization: juggle multiple priorities, meet deadlines, and adapt in a fast‑paced environment
Reliable transportation for client visits; valid driver’s license
