We’re currently seeking a detail-driven, highly organized Intake Coordinator to help with two of our busiest offices to streamline the journey from hospital to home for our patients.     Why This Role Matters:

    As an Intake Coordinator, you’ll be the first step in a patient’s home health care experience. Your precision, coordination, and communication skills will ensure every patient receives the care they need — when they need it. You’ll be the connection point between doctors, families, insurance providers, and our care team.

    What You’ll Do:

    • Own the intake process from start to finish — no detail too small.

    • Collect and organize medical, insurance, and personal information accurately and efficiently.

    • Work closely with hospitals, clinics, and healthcare providers to ensure smooth transitions into home care.

    • Assess patient needs and match them with the right services, treatments, and equipment.

    • Verify insurance coverage and manage authorizations.

    • Maintain precise electronic medical records and ensure all documentation is complete and compliant.

    • Be a compassionate point of contact for patients and families, answering questions and offering guidance during intake.

    What We’re Looking For:

    • At least 2 years of healthcare experience — Home Health a plus.

    • A strong grasp of medical terminology and EMR systems- we use Wellsky/Kinnser.

    • Meticulous attention to detail — you catch what others miss.

    • A calm, confident communicator who’s great with people and paperwork.

    • Comfortable working independently and with a supportive team.

    • A flexible, adaptable attitude — no two days are the same.

    Preferred Qualifications:

    • Associate’s or Bachelor’s degree in a healthcare-related field.

    • Familiarity with insurance verification and authorizations.

    • Bilingual skills are a plus!
