Green Meadows Hospice is looking for a Full Time experienced Intake Coordinator.
The hospice intake coordinator plays a crucial role in the admissions process for hospice care, ensuring that patients and families receive appropriate support and information.
Key Responsibilities:
Patient Assessment: Conduct initial assessments to determine eligibility for hospice services.
Intake Process: Manage the intake process, including gathering necessary documentation and medical records.
Communication: Serve as the primary point of contact for patients, families, and healthcare providers.
Coordination: Collaborate with interdisciplinary teams to facilitate care plans and services.
Education: Provide information about hospice services, benefits, and options to patients and families.
Compliance: Ensure adherence to regulatory requirements and organizational policies.
Skills and Qualifications:
Experience: Previous experience in healthcare, particularly in hospice or palliative care, is often preferred.
Interpersonal Skills: Strong communication and empathy skills are essential for interacting with patients and families.
Organizational Skills: Ability to manage multiple tasks and maintain accurate records.
Work Environment:
Hospice office. Hours 830am - 5pm Monday - Friday
