About the Role:

Join Sunrise Health Care, PC as a Family Nurse Practitioner (FNP-C) in beautiful Colorado Springs, CO! We are looking for a passionate and dedicated healthcare professional to provide exceptional patient care in a supportive and collaborative environment.

Responsibilities:

Conduct comprehensive patient assessments and develop individualized care plans.

Perform diagnostic tests and interpret results to guide treatment decisions.

Administer medications and provide patient education on health management.

Collaborate with multidisciplinary teams to ensure holistic patient care.

Maintain accurate and timely documentation of patient interactions.

Stay current with best practices and contribute to quality improvement initiatives.

Participate in community outreach programs to promote health awareness.

Provide compassionate support and guidance to patients and their families.

Requirements:

Current FNP-C certification and a valid Colorado nursing license.

Minimum of 3 years of clinical experience in primary care family practice or a related field.

Strong communication skills and a patient-centered approach.

Ability to work effectively in a fast-paced environment.

Commitment to continuous learning and professional development.

Experience with electronic health records (EHR) systems. EPIC experience helpful.

Demonstrated ability to build rapport with diverse patient populations.

Willingness to participate in team-based care and community initiatives.
