About the Position

An FQHC in Augusta, GA is seeking an experienced Nurse Practitioner or Physician Assistant to join our established outpatient practice. This role is ideal for a provider who is passionate about chronic disease management and family medicine and values long-term patient relationships.

• Minimum of 5 years of clinical experience required
  (New graduates cannot be considered for this role)

• Strong interest and experience in chronic care management and family medicine

• Full-time position

• Monday–Friday schedule (no weekends)
