    About the Position

    A Federally Qualified Health Center in Hammond, Louisiana is seeking an experienced Family Nurse Practitioner to join its team. This health center has a long-standing commitment to serving diverse and underserved populations across the region, offering high-quality, affordable care through a comprehensive, patient-centered model.

    Requirements:

    • 3–5 years of experience as a Nurse Practitioner

    • Active Louisiana NP license

    • Experience with both adult and pediatric patients

    Schedule:

    • Monday–Thursday: 8:00 a.m. – 5:00 p.m.

    • Friday: 8:00 a.m. – 2:00 p.m.

    • Saturday–Sunday: Closed

    Clinical Focus Areas:

    • Chronic disease management (Diabetes, Cardiovascular Disease, Asthma, COPD)

    • Preventive health services (Immunizations, Cancer Screenings, Wellness Checks)

    • Integrated Behavioral Health support

    • Same-day appointments for acute illnesses

    • Pediatric primary care

    Compensation & Benefits

    • Competitive Salary: $100,000 – $145,000 (commensurate with experience)
