    Qualifications:

    Required:

    • Associate degree or higher
    • 3 plus years of Registered Nurse (RN)
    • 3 plus years of experience in ANY of the following:
      • Geriatric Care
      • Senior Care
    • ALL of the following valid licenses/certifications:
      • California (CA) driver’s license, personal transportation, good driving record, and auto insurance
      • American Heart Association Basic Life Support (AHA BLS)
      • Registered Nurse (RN) in California (CA)
    • Possess full proficiency in speaking, reading, and writing in Spanish.

     

    Preferred:

    • 1 plus years of experience in ANY of the following:
      • In-Home experience
      • Enrollment

     

    Overview:

    Join our client's team as an Enrollment Registered Nurse for the PACE Program in Los Angeles. This individual will help to bring the PACE (Program of All-Inclusive Care for the Elderly) program to serve frail elders and disabled older adults in underserved communities across Los Angeles. The Enrollment Registered Nurse is a key player in the Outreach Team as we find and assess prospective participants for our PACE program. This person acts as a liaison between the IDT (Interdisciplinary Team) and the Outreach Team to ensure a smooth enrollment process for the prospective participants.

     

    Key Responsibilities:

    • Collaborate with Outreach Specialist and IDT to ensure successful enrollment.
    • Conduct home visits to evaluate the prospective participant for potential eligibility for the nursing home level of care.
    • Conduct home safety evaluations of the prospective participants’ home.
    • Participate in Enrollment Eligibility meetings to review the Level of Care recommendations.
    • Complete Nursing Facility Level of Care (NFLOC) assessments and submit to DHCS for review; update/revise the assessment as requested by DHCS.
    • Maintain accurate documentation and record all evaluations in CRM software.
    • Participate in all PACE Audits by DHCS and CMS
    • Support the Center Director to develop participant retention strategies.
    • Maintain knowledge and expertise in eligibility, enrollment, and program specifications for the PACE Program
    • Support business development and census growth by participating and assisting with outreach programs as needed.

     

    Schedule and Shift Details:

    • Monday through Friday, 8:00 a.m. - 5:00 p.m. Mostly in-field home appointments and occasional onsite work in a Los Angeles clinic up to 5 days per month.

     

    Travel:

    • 95% travel to patient homes
