Qualifications:
Required:
- Associate degree or higher
- Over 1 year of experience in ALL of the following:
- Acute Care
- Emergency Department (ED)
- Critical Care
- Burn Unit/Wound Clinic
- Oncology
- Medical-Surgical (Med-Surg)
- Home Health Care
- Valid Registered Nurse (RN) in the State of Ohio (OH)
- Valid American Heart Association Basic Life Support (AHA BLS)
- Full-Time Position
Preferred:
- Bilingual in Spanish
- RN License Number
- Specialized certifications you have.

Our client is seeking a full-time RN Case Manager for Home Health in West Cuyahoga County, OH.

Key Responsibilities:
- Provide direct or indirect skilled comprehensive nursing care to a specified (or variety) of age and diagnostic groups.
- Utilize principles of the nursing process to assist in data collection, planning, and evaluation of nursing care.
- Demonstrate nursing knowledge and skills in implementing prevention, therapeutic, and rehabilitative nursing care.
- Utilize effective interviewing techniques, set limits, and collect data based on a specific role.
- Based on specific roles, communicate and collaborate effectively with health care providers, third parties, patients, and families.
- Maintain awareness of the comfort and safety needs of patients
- Observe patients, record significant conditions and reactions, and notify supervisor or Physician of patient's condition and response to drugs, treatments, and significant incidents.
- Respond to life-saving situations based on nursing standards, policies, procedures, and protocol.
- Use oral and written communications to convey pertinent information to members of the healthcare team promptly.
