Qualifications:

Required:

• Doctorate degree.
• ALL of the following valid licenses/certifications:
• American Heart Association Basic Life Support (AHA BLS)
• American Heart Association Advanced Cardiac Life Support (AHA ACLS)
• Valid Pharmacist in the State of Oregon (OR)

Overview:

As our client's Clinical Pharmacist I, you will be responsible for providing a broad range of pharmaceutical care services to ensure safe and effective medication therapy for inpatient and outpatient settings. Collaborate with multidisciplinary care providers to deliver optimal medication therapy aligned with patient care needs and regulatory standards.

Key Responsibilities:

• Responsible and accountable for providing a broad range of inpatient and outpatient pharmaceutical care services to ensure safe, effective, and prompt medication therapy.
• Support centralized and decentralized services and deliver optimal medication therapy to patients with a broad range of disease states.
• Work with multidisciplinary care providers to monitor, assess, and provide clinical interventions to ensure medication use is aligned with patient care needs, evidence-based practices, established policies and protocols, and regulatory standards.

• Varied Shift 8-80 // 80-hours per two-week pay period
