Job Duties:

Actively demonstrates balance's core values and behaviors and conducts oneself in a manner that is consistent with these values.

Comply with regulatory agency standards and regulations and knows/adheres to all laws and regulations pertaining to patient health, safety and medical information.

Evaluate new patients.

Create and implement goal-oriented treatment plans. Establish goals that are functional, measurable and attainable.

Treat patients and conduct re-evaluations to alter the plans of care and update the goals as individually required.

Assists patient towards goal achievement through effective treatment plan and interventions. Develops home exercises and self treatment plans for the patient.

Provides education to patient and family, including diagnosis and prognosis of therapy.

Responsible for the accurate and thorough completion of, evaluation reports, progress notes/flow sheets discharge summary and any additional required documentation.

Performs other duties consistent with purpose of job as directed.

Report to the Clinical Manager as directed and to discuss and correct any potential patient care or documentation issues.

Job Requirements

Education and Licencing

Successful completion of a Bachelor's Degree required.

An active Physical Therapist license in the state of Indiana is required.

Required Skills

Pelvic Health and Wellness experience preferred.

Vestibular Rehabilitation and Balance Therapy experience preferred.

EMR or "Electronic Medical Records" knowledge is preferred.

Professional appearance and the ability to eloquently interact with patients and their family members.
