    POSITION SUMMARY: In-Home Therapist/Individual Skill Worker Provides community based, in-home mental health services for children and families in the Comprehensive Community Services (CCS) Program. Position works independently and in collaboration with other CCS workers, community agencies, and organizations. Position is responsible for assisting the care team lead in providing therapeutic interventions to patients and/or families who struggle with any combination of mental illness, behavioral conditions, AODA, and chronic disease management. Services are provided in the home and/or community setting and include skill building. Position works in compliance with all state statutes and regulating bodies, in accordance with Family Impact Program policies and procedures, and within the scope of training and skill level.

    POSITION RESPONSIBILITIES:
    • Execute treatment plan and measurable goals
    • Timely collaboration with referral and ancillary community agencies
    • Provide skill building to patients and families with a combination of mental illness and substance abuse or chronic diseases in a home, community, and school setting
    • Provide case management and/or referral as necessary
    • Educate patients, their support systems and staff on prevention and treatment enhancement techniques
    • Maintain documentation/records that accurately reflect the services provided and the patient’s growth towards treatment goals
    • Perform other duties as assigned
    DESIRED SKILLS AND EXPERIENCE:
    • Understanding of DSM-IV criteria
    • Must possess strong oral and written communication skills
    • Ability to exercise good judgment and discretion in establishing work schedule
    • Must establish and maintain productive and professional relationships with patients, staff and community partners
    • Ability to work independently, effectively manage time and maintain organizational skills
    • Possess creativity in identifying problems and creating solutions
    • Knowledge and compliance of all applicable guidelines, policies, procedures and laws
    • Ability to handle confrontational situations in a professional and productive manner
    • Experience with group therapy a plus
    • WI Substance Abuse certification a plus
    EDUCATION, TRAINING AND OTHER:
    • Current driver’s license and access to your own vehicle; mileage reimbursed
    • This position is highly independent, but has the resources you need to succeed and grown in your behavioral health career
    • This position is currently part-time with the potential to be full-time based off of good rapport.
