    Key Responsibilities:- Provide holistic family medicine services, including preventive care and effective management of acute and chronic conditions, ensuring the health and well-being of your patients.- Conduct in-depth assessments, craft personalized care plans, and ensure seamless continuity of care that caters to each patient’s unique needs.- Collaborate with a multidisciplinary team of healthcare professionals to provide integrated and effective healthcare solutions, reinforcing a culture of teamwork and support.- Educate and empower patients and their families with knowledge about health management, preventive strategies, and overall wellness.- Play an active role in quality improvement initiatives, helping to enhance the overall healthcare experience for our community.- Engage in community outreach to foster health education and to promote awareness of available services.- Uphold meticulous and timely medical record-keeping in line with organizational and regulatory requirements.
    Qualifications:

    Background and Education:

    Medical Degree from an accredited institution; New York State Medical license; New York State

    Medical Board Certification; minimum of two years’ clinical experience preferred; ability to speak Spanish or other languages preferred.

    - Deep understanding of the unique healthcare challenges faced by diverse and underserved populations.

    - Exceptional communication and interpersonal skills, with a knack for building rapport and trust.- A collaborative mindset and the ability to thrive in a team-oriented atmosphere.- The Physician will supervise mid-level practitioners in accordance with the terms of the collaborative agreement.
