    The Oasis Center of the Rogue Valley and Continuum Behavioral Health seek a full-time Family Medicine Provider (MD/DO/FNP)

    Full-time, 4 days/week (Monday – Friday) Electronic Health Record: EPIC, with Dax AI

    Continuum Behavioral Health is seeking a family medicine provider who wishes to be part of an exciting and innovative new partnership working to improve the lives of families affected by substance use disorders (SUD). The successful candidate will join an established family medicine group (Oasis Center) specializing in the care of families affected by SUD integrated within the context of a larger behavioral health organization. The position will provide the full spectrum of family medicine, including pediatrics, adult acute and preventive care, prenatal care and medications for substance use disorders.

    In the Fall of 2026 Continuum will open its new Family Treatment Campus which will provide in a single location a full spectrum of care, from residential SUD treatment, mental health, childcare and medical care focused on strengthening families and supporting children’s healthy development.

    This patient-centered medical home includes medical providers, behavioral health, community health workers and doulas, integrated with social support services including housing, childcare, medical-legal partnership, community multidisciplinary team, and employment support. This unique program will include on-campus residential treatment for mothers and fathers, primary care medical services, mental health and psychiatry, nutrition, recreation, therapeutic children’s services, and outpatient treatment.

    Qualifications:

    MD/DO/NP from accredited medical school

    Current State of Oregon licensure (or ability to obtain)

    Strong interpersonal communication skills

    Board certification in Addiction Medicine preferred, but not required

    The Oasis Center is a unique community-based primary care and addiction medicine practice caring for families affected by substance use disorders. We provide services not often found in other clinics: child watch, parenting education, as well as employment services, medical/legal partnership, and DHS Self-Sufficiency (SNAP, TANF) partnerships to support families in all aspects of their lives impacting health. Oasis is an Oregon Health Authority Nurture Oregon site integrating substance use disorder care with prenatal/postpartum and infant care, and is a rotation site for the Oregon Health Sciences University (OHSU) Addiction Medicine Fellowship and Family Medicine Residency.
