We are seeking a compassionate and patient-centered advanced practice clinician to support primary care and community-based outreach services in the Princeville and Mendota area. In this role, you will provide high-quality, evidence-based care to patients across the lifespan, helping improve access to health services and promoting better outcomes in the communities we serve.
Community Health Partnership of Illinois is a 501 c 3 not-for-profit that provides primary preventative health care to uninsured and underserved community residence throughout northern and central Illinois, providing access to medical, dental, behavioral health and linkage to other social service. Our focus is delivering quality, culturally responsive health care services to our patients, caregivers, and partners in the communities we serve.
At CHP the Family Nurse Practitioner (FNP) is responsible for providing primary care to patients of all ages, from infants to elderly individuals.
The duties of a Family Nurse Practitioner may include conducting physical examinations, ordering and interpreting diagnostic tests, diagnosing and treating common illnesses and injuries, prescribing medications, managing chronic conditions, providing preventive care and health education, and referring patients to specialists when necessary. FNPs also collaborate with other healthcare providers to ensure comprehensive and coordinated care for their patients.
In addition to clinical responsibilities, Family Nurse Practitioners may also be involved in conducting research, participating in quality improvement initiatives, and contributing to the development of healthcare policies and practices. FNPs must possess strong communication and interpersonal skills, as they work closely with patients and their families to provide compassionate and effective healthcare.
Qualified candidates for this position must hold a master's or doctoral degree in nursing, be licensed as a Registered Nurse, and have certification as a Family Nurse Practitioner. Prior experience working in primary care or a related field is typically required. FNPs must also stay current with advances in healthcare and maintain their licensure, CPR/BLS and other certifications through continuing education and professional development activities.
