    Job Title: Family Nurse Practitioner or Physician Assistant

    Position Summary:

    We are seeking a dedicated and compassionate Advanced Practice Provider (APP) to join our Comprehensive Hemophilia Treatment Center. The APP will provide both primary care and specialized care for patients with hemophilia, von Willebrand disease, and other inherited and acquired bleeding disorders across the lifespan. In this role, the APP will collaborate closely with physicians, nurses, social workers, physical therapists, and other members of the multidisciplinary care team to ensure holistic, patient-centered care.

    Position Type and Expected Hours of Work:

    3-4 days/week (Monday, Thursday, and Friday preferred).

    Outpatient clinic-based position with some inpatient consultation responsibilities.

    Monday–Friday 8:00am-4:30pm schedule with 7-10 days of on call coverage for patient support. May be asked to attend community events or outreach as needed.

    Qualifications:

    Master’s degree or higher in Nursing (NP) or Physician Assistant Studies (PA).

    Current Wisconsin state licensure and board certification as a Family Practice NP or PA.

    Minimum 2 years of clinical APP experience in internal medicine, urgent care, emergency care, or family practice required.

    Experience with outpatient infusion, hematology and/or bleeding disorder expertise ideal but not required.

    Knowledge of the EPIC EMR strongly preferred.

    Strong clinical assessment, diagnostic, and critical thinking skills.

    Excellent communication and patient education skills with a team-oriented mindset.

    Key Responsibilities:

    Perform comprehensive health assessments, histories, and physical exams for patients with bleeding disorders including primary care services, ensuring continuity and comprehensive management of overall health needs.

    Manage acute and chronic conditions related to both primary care and bleeding disorders including medication management, pre- and post-procedural planning, perform small in office procedures, and care for urgent health needs.

    Order, interpret, and evaluate diagnostic tests relevant to patient management.

    Develop, implement, and adjust individualized treatment plans in collaboration with hematologists and the care team.

    Provide education and counseling to patients and families on disease management, home infusion therapy, prevention of complications, and lifestyle considerations.

    Participate in patient and family teaching sessions, outreach clinics, and community education events.

    Collaborate with Hematologists, clinical care team, HOC pharmacy staff, and specialty pharmacy programs to ensure safe and effective medication management.

    Engage in quality improvement initiatives, clinical research, and data collection efforts to advance care for patients with bleeding disorders.

    Maintain accurate, timely documentation in compliance with institutional and regulatory standards.

    Maintain necessary CEU’s for position as required by the State of Wisconsin
