    Family Preservation Services (FPS) of North Carolina has collaborated with Pathways.

    FPS of NC is seeking a dynamic individual to join our team as an ACTT Masters Level QP provides direct and indirect services to adult consumers with severe and persistent mental illness and resulting significant functional impairment. This position provides recovery-oriented interventions desigend to sustain active commuity living on as independent a level as possible for each consumer. This position participates in assessmnent, treatment planning, case management, service coordination and referral, supportive counseling and skills development. The ACT Team QP must have a master’s degree and meet the status of a Qualified Professional according to 10A NCAC 27G .0104. Here are some of the duties: Plan, coordinate and oversee initial and ongoing assessment activities; Conduct or participate in comprehensive clinical assessments to include assessment of: psychiatric history (e.g., onset, course and effect of illness, past treatment and responses, and risk behaviors), mental status, and diagnosis; physical health and dental health; use of drugs or alcohol; education and employment; social development and functioning; activities of daily living (e.g., self-care, living situation, nutrition, money management); and family structure and relationships; Develop client’s PCP using Person-Centered Thinking. Revise and update client’s PCP as necessary per result of ongoing assessment activities; Organize and facilitate regularly scheduled treatment team meetings to facilitate person-centered planning, revisions and updates. Include formal and informal supports as appropriate; Develop and maintain cooperative and collaborative working relationships with members of the client system.

    Position requires the ability for a flexible schedule to meet the needs of the client

    QUALIFICATIONS

    Master’s Degree in the Human Services discipline

    Documented experience providing treatment to adults with severe and persistent mental illness

    All individuals must be credentialed as a QP by meeting the requirements specified for Qualified Professional status according to 10A NCACV 27.G.104

    NC valid driver’s license and automobile insurance

    Must be able and willing to work a flexible schedule that could include working early mornings, evenings and/or weekends per the consumers’ needs

    MUST MEET REQUIREMENTS FOR A QUALIFIED MENTAL HEALTH OR SUBSTANCE ABUSE PROFESSIONAL IN NORTH CAROLINA:

    Graduate of an accredited college or university with a Baccalaureate Degree in Social Work or Psychology, or a related human services field. Has 2 years of MH/DD/SA experience providing direct service to the population served. All individuals must be credentialed as a QP by meeting the requirements specified for Qualified Professional status according to 10A NCAC 27.G.0104.

    OR

    Graduate of an accredited college or university with a Master’s Degree in Social Work, Psychology, or a related human services field. Has 1 year of MH/DD/SA experience providing direct service to the population served OR has contracted with a professional licensure board. All individuals must be credentialed as a QP by meeting the requirements specified for Qualified Professional status according to 10A NCAC 27.G.0104.

    OR

    Graduate of an accredited college or university with a Baccalaureate Degree or a Master’s Degree in a field unrelated to human services. Has required number of years of MH/DD/SA experience providing direct service to the population served. All individuals must be credentialed as a QP by meeting the requirements specified for Qualified Professional status according to 10A NCAC 27.G.0104.
