    Family Preservation Services is a subsidiary of Pathways and specializes in non-residential, community-based services for children, adolescents, and adults with severe emotional/ behavioral disorders who are also receiving Medicaid. The goal of FPS is to provide “Human Services Without Walls,” meaning that services are mostly provided in the client’s own environment and community.

    SUMMARYServes as a member of the multi-disciplinary Assertive Community Treatment (ACT) Team. The ACT APRN is responsible for providing ethical quality, direct services to those with complex, severe and persistent mental illness. The ACT APRN provides culturally competent, medically necessary, treatment, rehabilitation, and support services to FPS consumers and collateral supports in compliance with state and federal regulatory standards. The ACT APRN provides services in the home, community and the office. ACT is a 24/7, team approach providing services 365 days a year. The ACT APRN provides services in the community that are consistent with core operating principles of professional nursing ethics, state and regulatory guidelines.

    ESSENTIAL DUTIES AND RESPONSIBILITIES• Monitoring of all medical/clinical aspects of treatment delivered to consumers• Provide psychiatric evaluations to include: comprehensive psychiatric history, mental status and diagnosis• Provide medication management and supportive therapy• Utilize the SAMHSA/ DACTS standards in monitoring fidelity to the ACT treatment model• Oversee quality management initiatives pertaining to medical services• Ensure ACT Team coordination with primary care providers and specialty healthcare providers• Consult with primary healthcare providers on the clinical aspects and the implications for consumers with physical healthcare needs• Coordinate with medical staff of other providers, e.g. state and community inpatient facilities• Provide medical oversight and supervision of nursing staff• Participate in the development and implementation of effective treatment plans• Develop and maintain effective, comfortable relationships with consumers and teammembers• Attend and actively participate in daily staff meetings (at minimum, one perweek)• Provide training, guidance and consultation to ACT Team Members• Complete all necessary documentation as required by FPS, DHB, DHH, Medicaid, other 3rd party Payers, and licensure forms in a timelymanner

    REQUIRED EDUCATION AND/OR EXPERIENCE• Current licensure as a Psych Advanced Registered Nurse Practitioner in the state of Louisiana

    OTHER REQUIREMENTS:• Must have documented experience providing treatment to adults with severe and persistent mental illness• Must possess a valid driver’s license with a safe driving record.• Must be able and willing to work flexible schedules to include working early mornings, evenings, and weekends per consumer need.

    PREFERRED EDUCATION AND EXPERIENCE• Bachelor’s or Master’s degree preferred. Has a minimum of two years of nursing experience and one year of specialized psychiatric experience• Has knowledge of in implementing
