    SUMMARY

    The Healthy Transitions Assessor will report to the Adult Enhanced Services Program Manager and/or designee, and will provide outreach and conduct assessments to promote service access for youth, young adults, and adults.

    TRAVEL

    This position requires travel as needed under outreach requirements and as possible under pandemic precautions.

    REQUIRED EDUCATION AND/OR EXPERIENCE

    Master’s Degree in Human Services

    A graduate of a college or university with a Master’s Degree in a human service field. The individual must hold an unrestricted license issued by a licensing board in one of the following disciplines: psychology, psychiatry, social work, or counseling.

    Or

    A graduate of an accredited college or university with a Master’s Degree in a human service field. The individual must be Provisionally Licensed by the Social Work Board or Board Eligible by the NC Counseling Board or an Associate Licensed Marriage and Family Therapist.

    PREFERRED EDUCATION AND EXPERIENCE

    Mental health or human services system

    Basic knowledge of Substance Use disorders and systems

    Child and Adolescent development/needs/behavior

    Child and/or Adult psychopathology

    Person-Centered Planning Philosophy

    System of Care Philosophy

    Evidenced-Based Best Practices

    Family systems and family dynamics

    Medical/Clinical and Medicaid documentation standards and practices

    Community resources, natural/informal supports

    Cultural Competence

    DSM-V

    Knowledge of mandated reporting requirements

    State Service Definitions

    3 years related experience and/or training, full licensure preferred

    Familiarity with the Transition to Independence Process (TIP) Evidence Based Practice

    CERTIFICATES, LICENSES, REGISTRATIONS

    Unrestricted license issued by a licensing board in one of the following disciplines: psychology, psychiatry, social work, or counseling, or, Provisionally Licensed by the Social Work Board or Board Eligible by the NC Counseling Board or an Associate Licensed Marriage and Family Therapist.
