    The CSII Treatment Foster Care Case manager provides direct care services to the child placing agency in Treatment Foster Care include: treatment plan development and implementation with individuals and families through the use of proper therapeutic and behavioral interventions and/or training skills. The CSII works independently in each clients’ home and other community settings, utilizing clinical interventions that are based on best practice models and learned techniques that work effectively with individuals and families.

    REQUIRED EDUCATION AND/OR EXPERIENCE

    Masters degree in Social Work or a related field (Psychology, Counseling, Sociology, Special Education, etc) PLUS a student placement in casework services to kids and families OR 1 year of case management experience working with kids and their families.

    OR Bachelors degree in Social Work or a related field (Psychology, Counseling, Sociology, Special Education, etc) and 1 year of case management experience working with kids and their families.

    OR a Bachelors degree in any field plus 2 years of case management experience working with kids and their families.

    Past Therapeutic Foster Care experience preferred

    Consent to all required criminal and CPS background investigations and TB assessment and/or test

    CERTIFICATES, LICENSES, REGISTRATIONS

    Possession of a valid driver’s license and verification of a safe driving record

    SUMMARY OF RESPONSIBILITIES

    Engage client and family in the development of an Individualized Service Plan (ISP). Establish and maintain effective and collaborative working relationships with all collateral community agency professionals. Participate in collaborative treatment planning and coordination of meetings for all clients, including those involving schools, other providers, foster parents, biological parents, DSS, CSA, court appearances, etc.

    Interviewing children and families. Provide individual, family, and group sessions utilizing the therapeutic interventions outlined in the Individualized Service Plan. Work to join appropriately with individuals and/or families in establishing effective professional relationship with an awareness of maintaining appropriate boundaries. Counseling children and families in preparation for placement or discharge.

    Document all services provided according to FPS/ VA licensure requirements and standards in the established timeframes. Preparing and maintaining case records. Conducting and writing home studies. Service planning by developing individualized treatment and service plans.

    Attend and actively participate in supervision meetings, treatment team, and required trainings.

    Supervising children in resource, foster, and adoptive homes; and in independent living arrangements;

    Coordinating services to minimize fragmentation of care, reduce barriers, and link children with appropriate services to ensure comprehensive, continuous access to needed medical, social, educational, and other services appropriate to the needs of the child. Monitoring and following up by assessing ongoing progress in each case and ensuring services are delivered. The case worker shall continually evaluate and review each child's plan of care. The case worker shall collaborate with the family assessment and planning teams and other involved parties on reviews and coordination of services to children and their families

    Coordinating referrals by assisting the child in arranging for appropriate services and ensuring continuity of care for a child in treatment foster care. The case worker shall link the child to services and supports specified in the individualized treatment and service plan. The case worker shall directly assist the child to locate or obtain needed services and resources. The case worker shall coordinate services and service planning with other agencies and providers involved with the child by arranging, as needed, medical, remedial, and dental services;

    Provides crisis response services which may include after-hours on-call responsibility.

    May carry a caseload of up to 12 TFC consumers.
