    Service Philosophy

    Family Preservation Services (FPS) provides services in the client’s own environment or human services without walls. FPS does not own or operate any hospitals or residential facilities. Clients are served in their homes, neighborhoods, and communities. FPS began offering services in the Washington, DC area in the summer of 2004. FPS is certified ad licensed by the District of Columbia Department of Behavioral health (DBH) as a specialty provider. FPS accepts referrals from all District agencies.

    FPS offers the following DBH Mental Health Rehabilitation Services:

    Community Based Intervention (CBI)

    CBI is offered in the child’s natural setting. This includes behavior support, case management, and referral to community-based sources of support. CBI services are provided to children and youth aged 6 to 21 years old and to their families.

    Assertive Community Treatment (ACT)

    ACT is a specialty service that involves a multidisciplinary team approach offered in the adult’s natural setting. ACT helps those with serious and persistent mental illness. ACT help those who have not been successfully engaged in services and/or those who have not benefited from traditional mental health services. ACT services are provided to individuals aged 25 and older.

    Transitional Assertive Community Treatment (TACT)

    TACT is a subset of ACT, serves consumers from 18-29. TACT help those who have not been successfully engaged in services and/or those who have not benefited from traditional mental health services.

    Family Preservation Services (FPS) is seeking a dynamic individual to join our team as Advanced Practice Psychiatric Registered Nurse (APRN). the APRN serves as a member of the multi-disciplinary Assertive Community Treatment (ACT) Team. The ACT APRN is responsible for providing ethical quality, direct services to those with complex, severe and persistent mental illness. The ACT APRN provides culturally competent, medically necessary, treatment, rehabilitation, and support services to FPS consumers and collateral supports in compliance with Mental Health Rehabilitation Services (MHRS) and federal regulatory standards. The ACT APRN provides services in the home, community and the office. ACT is a 24/7, team approach providing services 365 days a year. The ACT APRN provides services in the community that are consistent with core operating principals of professional nursing ethics, State, and regulatory guidelines.

    ESSENTIAL DUTIES AND RESPONSIBILITIES

    Provide brief individual medical psychotherapy designed to reduce maladaptive behaviors related to the individual’s behavioral health disorder, to maximize behavioral self-control, or to restore normalized functioning and more appropriate interpersonal and social relationships.

    Provide behavioral health status examinations which are clinical, psychiatric, diagnostic, or evaluative interviews to assess the behavioral stability or treatment status of the individual being served.

    Conduct AIMS, Stretcher, or other evaluations and reports as required. Complete FPS DC and/or DBH Incident Reports on critical incidents, including suicide attempts, deaths, and the initiation of FD-12’s on individuals served, as necessary.

    Provides medication management services including evaluation of the need for medication, evaluation of the clinical effectiveness and side effects of medication, prescribing medications, and medication education with the individual being served and/or other responsible persons as appropriate concerning the risks, benefits, and alternatives to medications. (This includes meeting the consumer where they are. i.e. community, homeless shelters, etc.)

    Provides on-going assessment of nursing and psychiatric services to support integrated treatment needs. Conduct the medical portion of the client-centered comprehensive diagnostic assessment of psychiatric history (as determined).

    Makes treatment recommendations and regimens as a part of the treatment team, develops individual integrated care plans for all consumers identified as high-risk.

    Facilitates ongoing consumer education on health prevention/promotion, diet, safer health behaviors, family planning, smoking cessation and recovery Involve family, significant others, and any other collateral designated by the consumer in health education services that addresses the consumers health needs.

    Participates in treatment team to coordinate and promote integrated physical, mental health and wellness services. Effectively network with other community agency personnel to integrate practices.

    Routinely consult with the psychiatrist and clinical management team regarding treatment needs and outcomes.

    QUALIFICATIONS

    Current licensure as a Psych Advanced Registered Nurse Practitioner in the District of Columbia

    Bachelor’s or Master’s degree preferred. Has a minimum of two years of nursing experience and one year of specialized psychiatric experience, preferred.

    Has knowledge of and experience in implementing MHRS regulations, policies and procedures.

    Must have a valid, unrestricted driver’s license and reliable transportation.
