    About the Role

    Join Paving the Way Multi-Service Institute (PTW) as a Family Support Worker (FSW) – Case Manager and make a meaningful impact in the lives of children and families in Washington, DC. This is an in-person, community-based role where you will work directly with families in their homes and communities to build stability, prevent unnecessary foster care involvement, and ensure children remain safe and supported with their families.

    Each FSW carries a caseload of about 15 families to ensure high-quality, individualized services. The work requires flexibility, cultural humility, and a commitment to being available for occasional late-day or after-hours emergencies.

    Key Responsibilities Case Management & Planning

    Conduct comprehensive family assessments to identify strengths, needs, safety, and risk.

    Develop, update, and monitor individualized Prevention Plans and Family Development Plans.

    Provide referrals, coaching, and advocacy to connect families with community resources and social services.

    Track progress and adjust plans to meet evolving family needs.

    Family Engagement

    Conduct required home visits and engage directly with children in the home at least twice per month.

    Build supportive, trust-based relationships with parents, caregivers, and children.

    Support kinship, guardianship, and reunification situations to keep children safely at home.

    Facilitate workshops or support groups as needed.

    Documentation & Compliance

    Document all activities, case notes, and service plans in the electronic health record (EHR) system within required timelines.

    Maintain accurate, timely, and family-centered documentation that meets PTW and CFSA quality standards.

    Ensure compliance with federal, state, and local regulations.

    Collaboration & Professional Development

    Partner with schools, CFSA, community providers, and PTW Clinical Supervisors to coordinate services.

    Participate in weekly individual clinical supervision (1 hour) and monthly group supervision to support professional growth.

    Remain flexible for after-hours coverage as needed.

    Qualifications

    Required:

    BA/BS degree in Social Work, Psychology, or related human services field.

    At least 1–2 years of experience working with children and/or families (community-based or nonprofit strongly preferred).

    Demonstrated ability to work independently while contributing to a team.

    Preferred:

    Strong knowledge of child welfare systems, family preservation, and community resources.

    Proficiency with Microsoft Office and case management/EHR software.

    Excellent organizational, written, and interpersonal communication skills.

    Compassionate demeanor and a commitment to family-centered, strengths-based practice.
