We are seeking dedicated Registered Nurses (RNs) who are passionate about providing exceptional home health care to Adult Geriatric, Women's Health, and Pediatric populations.
Position Type: Per Diem (Fee for Service)
Compensation:
- $85 per routine visit
- $105 per recertification/discharge
- $120 per admission
Responsibilities:
- Provide skilled nursing care to patients in their homes across Adult Geriatric, Women's Health, and Pediatric populations.
- Perform comprehensive patient assessments and develop individualized care plans.
- Administer medications, treatments, and other physician-ordered interventions.
- Complete admissions, recertifications, and discharge visits.
- Educate patients and caregivers on diagnoses, medications, treatment plans, and disease management.
- Monitor patient progress and communicate changes in condition to the appropriate healthcare providers.
- Maintain accurate, timely documentation in accordance with agency, state, and federal regulations.
- Collaborate with physicians, therapists, social workers, and other interdisciplinary team members to ensure quality patient care.
Qualifications:
- Active Registered Nurse (RN) license in New York.
- Experience in Adult Geriatric, Women's Health, and/or Pediatrics preferred.
- Home health or community nursing experience is a plus.
- Strong clinical judgment, assessment, and communication skills.
- Ability to work independently and manage a flexible schedule.
- Reliable transportation to travel throughout Brooklyn and Queens.
