    About the Position

    The Personal Care Coordinator (Bright Steps) will coordinate activities aligned with the Certified Perinatal Services Program (CPSP) and support CalOptima Health moms with trimester specific, postpartum assessments and infant assessments. The incumbent will ensure communication of the member’s assessment and care plan with the member and the health care team. The incumbent will identify barriers to member’s care and assist in improving these barriers for all levels of care. The incumbent will work closely with the health care team to ensure member access to timely services and coordination of care.

    Duties & Responsibilities:

    • Participates in a mission-driven culture of high-quality performance, with a member focus on customer service, consistency, dignity and accountability.
    • Assists the team in carrying out department responsibilities and collaborates with others to support short- and long-term goals/priorities for the department.
    • Provides outreach to members to coordinate completion of trimester specific assessments, including postpartum following CPSP protocols.
    • Conducts telephonic assessments.
    • Collaborates with the health care team to ensure timely communication of member’s clinical information.
    • Maintains documentation of member’s case, care plan and communication with the health care team within CalOptima Health’s medical management system and ensures appropriate documentation of all interventions.
    • Identifies the need for referrals and coordinates with community resources or behavioral health services as needed.
    • Facilitates warm transfers in accordance with the member’s needs when appropriate and notifies member’s care team of key event triggers.
    • Guides members in understanding and accessing the benefits they are entitled to under Medi-Cal.
    • Maintains compliance with established departmental productivity guidelines.
    • Provides productivity metrics reports to supervisor as needed.
    • Works with CalOptima Health staff from other departments to expedite the resolution of member concerns.
    • Completes other projects and duties as assigned.

    Experience & Education:

    • High School diploma or equivalent required.
    • 1 year of experience working with maternal and child health programs required.
    • Bilingual in English and in one of CalOptima Health's defined threshold languages (Arabic, Farsi, Chinese, Korean, Spanish, Vietnamese) required.
    • An equivalent combination of education and experience sufficient to successfully perform the essential duties of the position such as those listed above may also be qualifying.

    Preferred Qualifications:

    • Certification from California Board of Medical Assistant or American Registry of Medical Assistants.
    • 2 years of experience working with maternal and infant health programs

    • Physical demands: While performing duties of job, employee may be required to move about the organization. Employee must be able to sit for extended periods of time, as well as work at the computer for long periods. Must also be able to reach with hands and arms and must occasionally lift office supply boxes or laptop case, up to 25 pounds. Employee must be able to communicate, particularly for regular phone use, in meetings, face-to-face interaction and while presenting.

    • Work Environment: Typical office environment with minimal to moderate noise levels and controlled office temperatures.

    • Ensure that the estimating process runs smoothly and efficiently, and client expectations are exceeded throughout the duration of the project(s).
    • Prepare quantity take-offs and apply unit pricing for material and labor to establish a value for the work.
    • Prepare qualifications and assumptions for the estimate.
    • Prepare cost comparisons/reconcile with previous estimates.
