Responsibilities: • Coordinate and manage comprehensive care plans for hospice patients, ensuring their unique needs are met with compassion and precision. • Conduct regular assessments to monitor patient progress and adjust care plans as necessary, fostering a responsive and adaptive care environment. • Collaborate with interdisciplinary teams to deliver holistic care, ensuring seamless communication and coordination among healthcare providers. • Educate patients and their families about hospice care processes, empowering them with knowledge and support during challenging times. • Provide emotional and psychological support to patients and families, helping them navigate the complexities of end-of-life care with empathy and understanding. • Maintain accurate and timely documentation of patient care activities, ensuring compliance with healthcare regulations and standards. • Advocate for patients' rights and preferences, ensuring their voices are heard and respected in all aspects of their care journey. Qualifications: • Experience in hospice or palliative care, demonstrating a deep understanding of end-of-life care needs. • Ability to develop and manage comprehensive care plans tailored to individual patient needs, ensuring compassionate and precise care delivery. • Proven track record of collaborating effectively with interdisciplinary teams to provide holistic and coordinated care. • Strong communication skills to educate and empower patients and their families about hospice care processes. • Demonstrated ability to provide emotional and psychological support, helping patients and families navigate end-of-life complexities with empathy. • Proficiency in maintaining accurate and timely documentation, ensuring compliance with healthcare regulations and standards. • Ability to advocate for patients' rights and preferences, ensuring their voices are respected in all aspects of care delivery. Compensation: $53 - $56 hourly
