    Field RN Supervisor

    New York City (Field-Based)

    Position Overview

    We are seeking an experienced Field RN Supervisor to lead and oversee skilled nursing services in the field within a regulated home care environment. This role is both clinical and supervisory, combining hands-on patient care oversight with staff training, performance management, and regulatory compliance. The Field RN Supervisor plays a critical role in ensuring high-quality care delivery, staff competency, and adherence to DOH and accreditation standards.

    The Ideal Candidate

    • Licensed RN with strong clinical judgment and leadership presence

    • Experienced in home care or community-based nursing environments

    • Confident managing nurses, caregivers, and interdisciplinary teams

    • Organized, detail-oriented, and compliance-driven

    • Comfortable handling audits, incident reviews, and regulatory matters

    • Approachable mentor who supports staff development and accountability

    Key Responsibilities

    • Train, onboard, and supervise field RNs, including trial period oversight

    • Assign cases and abstracts to nursing staff based on coverage and acuity

    • Conduct monthly internal audits to ensure regulatory and agency compliance

    • Serve as a clinical and compliance resource for field-based teams

    • Partner with HR and coordination teams on training and performance matters

    • Conduct performance evaluations and ongoing competency assessments

    • Provide patient and family education, guidance, and emotional support

    • Maintain accurate, timely clinical documentation and patient records

    • Manage incident reporting, follow-up, and resolution

    • Handle DOH complaints and ensure timely corrective actions

    • Lead emergency preparedness drills and required in-service trainings

    • Maintain and update clinical policies and procedures

    • Support accreditation readiness and participate in audits as needed

    • Collaborate with leadership to implement workflow improvements

    Qualifications & Must-Haves

    • Active New York State Registered Nurse (RN) license

    • Graduate of an accredited nursing program (Diploma, BSN, or MSN)

    • Clinical nursing experience; home care strongly preferred

    • Minimum 2 years of supervisory or management experience

    • Strong assessment, documentation, and decision-making skills

    • Working knowledge of DOH regulations and home care compliance standards
