Field Registered Nurse (RN) – Home Care

New York, NY (travel required for home visits)

$50-55/hour

About the Role:

We are a growing home care agency seeking a compassionate, dedicated, and self-motivated Field RN to join our team. In this role, you’ll provide personalized in-home clinical care, develop care plans, and collaborate with families and team members to ensure the best outcomes for our clients.

Key Responsibilities:

• Conduct initial assessments and create individualized care plans based on patient needs.

• Perform routine in-home visits to monitor health status and adjust care plans as needed.

• Provide direct clinical care and ensure adherence to physician orders.

• Educate and support clients and families on health conditions and treatment plans.

• Supervise and train Home Health Aides and Personal Care Assistants.

• Maintain accurate and timely electronic documentation.

• Collaborate with the interdisciplinary care team during case conferences and care strategy meetings.

Qualifications:

• Valid New York State Registered Nurse (RN) license

• Minimum 1 year of clinical nursing experience (home care experience strongly preferred)

• Strong communication and interpersonal skills

• Compassionate, patient-centered approach

• Reliable transportation and valid driver's license

• Ability to multitask, problem-solve, and work independently

• Bilingual in Russian or Spanish a plus
