Field Supervisor Community Liaison Part Time Contract
Next Level Home Care, a Medicaid-certified and VA-credentialed home care agency, is seeking a reliable, professional Field Supervisor/Community Liaison to serve as our local representative in the Omaha metro area. Our leadership team is based out of state, so this person is our face in the community — someone clients, caregivers, and referral partners know and trust.
Schedule: 1–2 days per week, approximately 8 hours total (one full field day or two half-days). Flexible scheduling, but some visits will be unannounced spot checks, so availability across different days and times is important.
What you'll do:
Conduct unannounced pop-up visits to verify caregivers are on-site, on time, in compliance with care plans, and properly documenting (EVV clock-ins)
Perform client wellness check-ins — visit clients in their homes, assess satisfaction, identify concerns or changes in condition, and report back to the owner same-day
Complete new client intakes and assessments when needed, including paperwork, service agreements, and home environment review
Serve as the local point of contact for clients, families, and caregivers
Represent the agency at community events, senior centers, discharge planner meetings, and with referral sources as opportunities arise
Submit a brief written report after each field day (template provided)
What we're looking for:
Prior home care, home health, social services, or case management experience strongly preferred (CNA, LPN, RN, or social work background a plus)
Reliable transportation, valid driver's license, and auto insurance
Professional appearance and strong communication skills — you are the company in the room
Comfortable with documentation via phone or tablet (photos, forms, reports)
Able to pass background check and registry checks (APS/CPS, Nebraska Adult Protective Services registry)
Self-directed — you'll work independently with remote supervision
Compensation: $20–22/hr (1099 independent contractor) + mileage reimbursement at the IRS rate for travel between visits
