RN'S ONLY

We are looking for an Inpatient Care Coordinator to work with an awesome company who truly values its employees. In this position you will be responsible for the coordination of transitions and also identifying plans of care appropriate for each patients, in order to reduce length of stay and readmittance percentages.

You will follow patients post discharge from an acute care facility, while patients are being cared for in skilled nursing or rehab facilities. You will meet with patients, family members and the health care team to follow up on progress and resolve any barriers. You will have a caseload of about 25-30 patients.

You will start your day from home addressing any case management, provide patient support via phone, plan out site visits and any other tasks, as needed. Most of your day will be spent in the field; visiting patients onsite to do assessments and meet with the healthcare team. You will finish up at home.

Candidates must be willing to attend a 2-week training in Nashville (travel expenses are covered)

REQUIREMENTS

• Candidates must have an active license as an RN

• Five years of clinical healthcare experience

• At least 1 year (MIN) of recent case management experience within a skilled nursing facility

• Experience working with Interdisciplinary Teams

• An understanding of compliance with Medicare guidelines and procedures specifically for care within SNF
