Client location and schedule hours: Client is in the Belleview area The schedule is Mondays and Fridays 8:00 am to 2:00 pm. Job Responsibilities: Diagnoses: Afib, High Blood Pressure, Diabetes, Atherosclerosis, Post heart attack (bypass in 2002 with stints placed), Lower leg Edema, Venous Statious (leg blisters), Mild hearingloss without aids, & is slightly far-sighted.

ADL's:

Mobility: Client uses a walker at all times, but must be reminded if he starts to walk without it.. He can get up and down out of his chair independently. Client does not drive or have a car. CG may need to run errands such as take him for haircuts as needed.

Bathing/Dressing: Needs someone to come 2x per week to encourage & help him shower & wash hair. He needs prompts to change into clean clothes and may need assist with dressing, but CG needs to ask if he'd like help.

Medications: Remind him of his morning medications.

Toileting: Client is NOT incontinent. Brushes own teeth but needs reminders.

Eating/Drinking: groceries are delivered to his home. He can heat up and very minimally prepare his own food, but does not cook. He can feed himself and drink by himself.

Qualifications: Minimum of 1 year experience as an in-home Caregiver, CNA or related experiences with a loved one of family member Ability to effectively communicate through written and verbal language Valid Driver's License and adequate auto insurance Consent to criminal, elder abuse, motor vehicle and sex offender background checks Consent to drug and alcohol testing TB test
