🩺 LPN – Home Care Admission & Assessment Nurse (Paid Per-Visit Role)

Position Summary

Caregivers Unlimited is seeking a Licensed Practical Nurse (LPN) to complete in-home admission and assessment visits for clients receiving non-skilled home care services. This role focuses on initial admissions, supervisory visits, and care plan setup to ensure services are appropriate, authorized, and compliant with South Carolina agency policies.

This is a PAID per-visit position, ideal for nurses seeking flexible scheduling with predictable responsibilities and mileage reimbursement.

Key Responsibilities: Admissions & Initial Visits:

Complete and explain admission paperwork with the client or responsible party

Confirm authorized services, schedule, and caregiver expectations

Ensure documentation is accurate, complete, and signed

Communicate any concerns to the office

Assessments & Care Planning (Non-Skilled)

Develop or review task lists aligned with the service authorization and plan of care

Ensure all services remain within non-skilled scope (CLTC, CPC, Private Pay)

Supervisory & Follow-Up Visits:

Complete required and ongoing supervisory visits as assigned

Identify concerns and report changes to the office promptly

Documentation & Compliance:

Document all visits timely and accurately in the agency system

Maintain HIPAA compliance and confidentiality at all times

Communicate concerns, incidents, or changes in client condition to agency leadership

(This role is strictly oversight, assessment, and documentation.)

Schedule & Flexibility:

Visits are often grouped by area, allowing multiple visits in one trip

You set your availability

Compensation:

Flat rate per completed visit (initial, 30-day, or 90 day supervisory visit etc.)

Mileage reimbursement

Paid Bi-weekly per visit upon completion and submission of required documentation

Qualifications:

Active South Carolina Nurse license in good standing

Reliable transportation and valid driver’s license

Ability to travel for home visits

Strong documentation and communication skills
