The successful candidate will be a bilingual, caring, culturally-sensitive social services professional who has a positive influence on others, thrives on successfully facilitating processes, works at a faster-than-average pace, is extremely disciplined, detail-oriented, results-oriented, sets high standards for self and others, and demonstrates top-notch planning and coaching skills.
The ideal candidate will be excited about the opportunity to help IMPACT clients outline and execute individualized plans to move forward to self-sufficiency; and to coordinate some programs and workshops which are particularly relevant to improved lifestyles and behaviors. This person works with clients, volunteers, and staff; and reports to the Program Director.
DUTIES AND RESPONSIBILITIES (Work may vary depending on the organizations’ and client needs and will be communicated to the candidate/employee by leadership)
Facilitate the intake and interview processes of new clients, assessing their needs and assets.
Collaborate with Program Director to prioritize participants and their needs.
Assist participants with application processes for mainstream benefits.
Provide crisis intervention and referrals for program participants with physical or mental health concerns, substance abuse issues, or other special needs.
Develop and utilize a network of other social service providers offering programs appropriate for our IMPACT client participants.
Coordinate and facilitate ancillary IMPACT programs and workshops that will be helpful to clients in their journey to stability and self-sufficiency. Specific programs and workshops will be outlined each year in the job performance metrics section.
Make appropriate referrals in a timely manner.
Provide guidance and support to help the client meet pathway benchmarks.
Work with client families and individuals to outline individualized pathways to stability and eventual self-sustainability; monitoring progress on a regular basis, encouraging creative problem-solving and positive, productive progress.
Maintain case files on program participants, monitor and document participant needs, progress, and success, keeping information accurate, timely, and confidential.
Work with the Program Director to create a simple matrix assessment tool to measure access to and progress along 5-7 key elements of basic needs, and administer it to clients on a regular basis as required in the job description reporting metrics.
Enter data accurately, and in a timely manner, into the IMPACT database, as well as others such as Link2Feed, HMIS/Service Point, and as required by funders and leadership.
Follow all policies and procedures; adhere to any contractual agreements applicable to case management program.

KNOWLEDGE, SKILLS, AND ABILITIES
Knowledge of issues faced by low to moderate income participants, including some knowledge of the sociological, behavioral and cultural factors influencing their behavior and attitudes
Knowledge of motivational interviewing principles
Some knowledge of local services and community resources
Empathy, self-determination, advocacy and follow-up to see others succeed.
Knowledge of the principles, practices and techniques of case management and delivering programmatic services to participants
Sensitivity to and experience working with ethnically, culturally, socially and sexually diverse participants, communities, agencies and organizations.
Ability to organize tasks and establish priorities in response to changing conditions and in emergencies.
Ability to assess, discuss and document a participant's physical and mental abilities, financial, social and environmental needs, and to jointly formulate an effective success plan.
Ability to establish and maintain effective working relationships with co-workers, participants in adverse circumstances, and community services providers
Ability to communicate effectively orally and in writing
Computer literacy: Microsoft office products, Windows, Email, internet, database systems, etc
MINIMUM QUALIFICATIONS
Bilingual ( English/Spanish)
One year of experience as a case manager/resource specialist working with people with low income
Bachelor’s degree in a human services related area, BSW preferred
NOTE: an equivalent combination of related experience, education, and training may satisfy the above.
