    THE LANDING AT STONE OAK IS A WELL RESPECTED MEMORY CARE RETIREMENT COMMUNITY IN NORTH SAN ANTONIO CONVENIENTLY LOCATED OFF HUEBNER RD, NOT FAR FROM 1604 AND BLANCO RD.  WE HAVE AN OVERNIGHT (10PM- 6AM) LVN OPEN POSITION.

    General Description 

    Provides direct and individual nursing care to assigned residents based on nursing standards and under the supervision of the Director of Resident Care (DRC). Also maintains compliance with all state and federal regulations and the standards of the company

    Primary Duties and Responsibilities

    • Ensures quality and safe delivery of home health services to residents
    • Implements plan of care formulated by physician and DRC
    • Provides accurate and timely documentation of care given
    • Assesses and provides resident and family/care giver information pertinent to diagnosis or plan of care
    • Administers medication per physician orders. Documents properly on MAR. Observes for any adverse reaction and notifies physician accordingly
    • Assembles resident charts, resident records and checks that all necessary forms are complete and accurate
    • Prepares incident/accident reports for residents as needed per policy. Notifies physician and family post incident/accident per policy and documents appropriately
    • Maintains safe and healthy working environment and uses safe work methods and procedures in accordance with clinical standards
    • Assists resident in learning appropriate self-care activities
    • Organizes and monitors employees and material resources necessary for meeting the needs of the residents under his/her care
    • Recognizes emergencies and life-threatening situations and initiates appropriate action(s)
    • Maintains confidentiality and resident rights regarding all resident personal information
    • Performs any other duties requested by supervisor
    • Encourages teamwork and promotes company philosophy
    • Attends required community meetings
    • Is prompt and able to perform the required duties of the position on a regular, predictable basis
    • Becomes familiar and understands the steps for fire evacuation
    • All employees are responsible for maintaining a safe and secure environment for all community residents

    Knowledge, Skills, and Abilities

    • Able to read, write, speak and understand the English language. Bi-lingual a plus
    • Able to communicate effectively with all levels of management, employees, residents, family members, and outside contacts
    • Experience with Microsoft Office and Outlook software preferred
    • Able to make independent decisions
    • Able to seek out new methods and willing to incorporate them into existing practices when applicable
    • Must possess a passion to work with and around senior citizens

    Education Requirements

    • Graduated from an accredited program for LPN and is currently registered in the state of the community
    • Must obtain valid Continuing Education credits as required by the state of the community
    • Training Alzheimer and Dementia with focus on Validation Therapy

    Work Experience Requirements

    • Minimum of one (1) year nursing experience in a health care facility.
    • Experience in a long-care community and/or geriatric nursing preferred
    • Experience in emergency care and first-aid

    Physical Requirements

    The following physical demands are representative of those that must be met to successfully perform the essential functions of this position:

    • Stand and/or walk up to eight (8) hours a day
    • Sit up to 25% of the work day
    • Occasionally assist in lifting and/or transferring residents weighing up to 250 pounds
    • Frequently kneel, bend, reach, push, climb and/or balance stoop or kneel, use hands or fingers, reach with hands and arms, talk and/or hear
    • Close vision, distance vision and peripheral vision is required
    • Subject to infectious diseases, substances, odors, etc. throughout the work day.
