    GENERAL DESCRIPTION:

    Develops and facilitates programs to help meet the intellectual, spiritual, physical, psychosocial and recreational needs of the residents. Provides a variety of activities/classes including afterschool programs, scholastic tutoring sessions, recreational activities, health and nutrition resources, computer classes, social events and financial literacy classes in order to promote personal and community development.

    PRIMARY DUTIES AND RESPONSIBILITIES:

    • Provides general informal case management (intake and referral) to residents requesting such assistance when case manager is not available.
    • Assesses health, psychological and social needs. Develops an individually tailored case plan for services and periodic reassessment of the resident’s situation.
    • Establishes linkages with all agencies and service providers in the community: shops around to determine/develop the best “deals” in service pricing to assure individualized, flexible and creative services for the involved resident(s).
    • Sets up a directory of providers for use by both staff and residents.
    • Refers and links the residents to service providers in the general community, for example, case management, personal assistance, homemaker, meals on wheels, transportation, counseling, visiting nurse, preventative health screening/wellness and legal advocacy.
    • Educates residents on service availability, application procedures, client rights, etc. providing advocacy as appropriate.
    • May develop case plans in coordination with assessment services in community.
    • Monitors the ongoing provision of services from community agencies and keeps the case management and provider agency current with the progress of the individual.
    • May set up volunteer support programs with service organization in the community.
    • Helps residents build informal support networks with other residents, family and friends.
    • May provide training to residents in the obligation of tenancy or coordinate such training.
    • Presents workshops or informative sessions to residents on subject of resident interest.
    • Schedules recurring activities along with special events and holiday-specific events to enrich the daily lives of community residents. Ensures a mix of small-group and large-group to fulfill a range of human needs, from mental stimulation to creativity to physical fitness.
    • Creates printed monthly activities calendar and community newsletter.
    • Provides one or more forums (e.g. resident council) for residents to meet regularly and share their ideas, thoughts, and concerns regarding life enrichment programs and other community operations.
    • Maintains an adequate inventory of equipment and supplies for activities, keeping within budgetary guidelines.
    • Maintains activity participation records.
    • Manages monthly expense budget
    • Attends required trainings and meetings.
    • Becomes familiar with emergency response procedures, including fire evacuation.
    • All employees are responsible for maintaining a safe and secure environment for all community residents.
    • Performs other duties as assigned.

    KNOWLEDGE, SKILLS, AND ABILITIES

    • Able to read, write, speak and understand the English language. Bi-lingual in Spanish a plus.
    • Able to work independently as well as collaboratively with residents, agencies and the general community.
    • Able to communicate effectively with all levels of management, employees, residents, family members, and outside contacts.
    • Prompt and reliable and able to perform the required duties on a regular, predictable basis.
    • Clean driving record with license to operate Class B vehicle (or ability to obtain such license).
    • Able to make independent decisions.
    • Able to seek out new methods and willing to incorporate them into existing practices when applicable.
    • Must possess a passion to work with older adults, families with children and/or disabled persons.
    • Able to follow missing person policy and procedure and understand the Safe Return program.

    EDUCATION REQUIREMENTS

    • Bachelor’s degree in social work, psychology or related field preferred, or high school diploma with at least 3 years of related experience working with the families with children, disabled, and/or older adult population Working knowledge of supportive services and other resources for older adults and/or persons with disabilities is preferred.

    WORK EXPERIENCE REQUIREMENTS

    • Minimum of two years’ experience in social work, resident services, multifamily housing or another related field.
    • Experience in volunteer recruitment and training preferred.

    PHYSICAL REQUIREMENTS

    The following physical demands are representative of those that must be met to successfully perform the essential functions of this position:

    • Stand, walk, sit, push, climb and/or balance, bend, stoop, kneel, use hands and fingers, reach with arms, talk and/or hear throughout the work day.
    • Close vision, distance vision and peripheral vision is required
    • Subject to infectious diseases, chemical substances, odors, etc. throughout the work day.
