We are looking for a Registered Nurse to join our home health and outpatient team. The ideal candidate will have a passion for providing quality care to patients in a home health setting, and be able to demonstrate excellent communication and organizational skills. The successful candidate will be responsible for providing assessments, developing care plans, administering medications, and monitoring patient progress.

Responsibilities:

• Assess patient health needs and develop a plan of care.
• Administer medications and treatments as prescribed by a physician.
• Provide health education to patients and families.
• Perform home health visits and provide nursing care in the home setting.
• Provide patient assessment, monitoring, and evaluation of outcomes.
• Document patient progress and care in accordance with regulations and standards of practice.
• Coordinate with other healthcare professionals to ensure quality care for patients.

Salary: $75.00 - $85.00 per hour
