Position Summary:

We are seeking a full-time, reliable, and compassionate Physical Therapist to deliver one-on-one home health care to patients in the Frisco and Carrollton areas. The ideal candidate is experienced, self-motivated, and passionate about helping patients achieve their goals in the comfort of their own homes.

Key Responsibilities:

Evaluate, develop, and implement personalized treatment plans

Deliver direct physical therapy services in patient homes

Maintain accurate and timely documentation aligned with Joint Commission standards

Collaborate closely with the Director of Nursing

Supervise and mentor Physical Therapy Assistants (PTAs)

Qualifications:

Degree from an accredited Physical Therapy program

Current Texas PT licensure

Minimum 1 year of experience as a Physical Therapist (home health preferred)

Reliable transportation and ability to travel between patient locations

Benefits:
