Position Summary

We are seeking a Full-Time Occupational Therapist (OT) to join our home care team. In this role, you will provide client-centered, in-home occupational therapy services, helping individuals improve their ability to perform daily activities, enhance independence, and improve overall quality of life—within the comfort and safety of their homes.

Key Responsibilities

Initial assessment/Evaluate clients’ functional abilities and therapy needs in the home setting, in accordance with physician orders.

Develop, implement, and regularly update individualized treatment plans focused on ADLs, safety, and independence.

Provide hands-on therapy and educate clients and families on adaptive techniques, energy conservation, and home safety.

Recommend and train clients on adaptive equipment and assistive devices to support independence at home.

Collaborate with interdisciplinary team members including RNs, PTs, caregivers, and physicians to ensure coordinated care.

Maintain timely, accurate documentation of evaluations, progress notes, and discharge summaries in compliance with state and agency standards.

Deliver compassionate, respectful, and culturally sensitive care to diverse client populations.

Qualifications

Current, valid Massachusetts Occupational Therapist (OT) license.

Valid driver’s license and reliable transportation.

Current CPR certification.

Minimum 2 year of OT experience (home care experience is required).

Strong communication, organization, and time-management skills.
