Job Summary
Boundless Home Health is looking for a compassionate and dependable Full-Time Registered Nurse (RN) to join our team and provide care to patients throughout the Fort Worth area.
This is a field-based home health position, so the RN will travel to patients’ homes to provide skilled nursing care, complete assessments, monitor patient progress, and help manage individualized plans of care.
We’re looking for someone who is organized, communicates well, works independently, and genuinely cares about providing quality care to patients in their homes.
Responsibilities
Provide skilled nursing care to patients in their homes throughout the Fort Worth area
Complete patient assessments and monitor changes in condition
Develop, implement, and update individualized plans of care
Coordinate care with physicians, therapists, caregivers, and other members of the healthcare team
Administer medications and treatments as ordered
Provide wound care and other skilled nursing services as needed
Educate patients and caregivers regarding medications, treatments, disease management, and safety
Document visits and patient progress accurately and within required timeframes
Report significant changes in patient condition to the appropriate provider or supervisor
Maintain compliance with agency policies, clinical standards, and applicable regulations
Qualifications
Current and active Registered Nurse (RN) license in the State of Texas
Previous nursing experience required; home health experience preferred
Strong clinical assessment and documentation skills
Excellent communication and interpersonal skills
Ability to work independently and manage a full patient schedule
Valid driver’s license and reliable transportation
Current CPR certification
Must currently reside in or be able to regularly service the Fort Worth/DFW area
About the Position
This is a full-time, in-person home health nursing position serving patients primarily in the Fort Worth area. Applicants must be comfortable traveling to patients’ homes throughout their assigned territory.
