    We are seeking an energetic and driven Care Coordinator to join our growing home healthcare team. This role is ideal for someone who enjoys networking, marketing, relationship-building, and helping families connect with quality care services.

    This is not your typical desk job — you'll be out in the community meeting with hospitals, rehab centers, assisted living facilities, physician offices, case managers, and families throughout Palm Beach County.

    What You'll Do:

    • Build and maintain relationships with healthcare professionals and referral sources
    • Visit hospitals, rehabs, physician offices, and senior communities
    • Help coordinate care services for new and existing clients
    • Assist families through the intake and onboarding process
    • Handle paperwork, documentation, and communication with office staff
    • Represent the company professionally in the community
    • Help grow the company through outreach and relationship development

    What We're Looking For:

    • Excellent communication and people skills
    • Self-motivated and highly organized
    • Comfortable driving throughout Palm Beach County
    • Sales, marketing, healthcare, or customer service experience preferred
    • Home healthcare or medical industry experience is a plus
    • Reliable transportation required
