Now Hiring: Full-Time LVN – Hospice

Location: San Antonio, TX Schedule: Full-Time | Monday–Friday (8:00 AM–5:00 PM) Setting: Hospice – Patient-Centered Care

Mi Casa Hospice is seeking a compassionate and dependable Licensed Vocational Nurse (LVN) to join our growing hospice team. This role is ideal for an LVN who is passionate about end-of-life care and values providing comfort, dignity, and quality care to patients and families.

Responsibilities:

Provide skilled nursing care to hospice patients under the direction of the RN Case Manager and Medical Director

Perform patient assessments, vital signs, and symptom monitoring

Administer medications and treatments as ordered

Assist with wound care, catheter care, and other clinical tasks

Coordinate care with interdisciplinary team members

Educate patients and families on disease process, comfort measures, and medications

Document all visits accurately and timely in the EMR system

Follow hospice regulations and company policies

Qualifications:

Current Texas LVN license (required)

Hospice or home health experience (required)

Strong clinical and time-management skills

Compassionate, patient-focused approach to care

Reliable transportation and valid driver’s license

Ability to work independently and as part of a team
