    Qualifications:

    Masters degree from a school of Social work accredited by the counsel on Social work education.

    Knowledge of terminally ill patients and their families along with Hospice understanding and the psycosocial dynamics of illness , loss , and death.

    Minimum of 1 year of Social work experience in a health care setting.

    Strong time managemtn skills and ability to meet critical deadlines for charting.

    Familiarity with community resources and support services for patients and families.

    Demonstrated experience and public speaking

    Demonstrated ability to work independently while functioning as part of a coorperative and coordinated team

    Must possess excellent interpesonal skills, effective organizational, sound judgement, prioritization, and follow- thru skills, attention to detail, tact, emotional intelligence and the ability to promote positivity.

    Responsibilities:

    As a MSW, you will be responsible for the provision of consultation and service to homebound individuals and the need of assistance with the solution of complex social and emotional problems, which impede management of medical regimen

    Provided appropriate interventions to patients and families during times of emotional, psychosocial or financial stress as related to their medical conditions

    Monitors length of stay and identifies and particiaptes in the development and implentaion of strategies to reduce unneccassry resource consumption on assigned patient populations.

    Keeps abreast of current changes in law which effect Social work practice.

    Works closely with physicians, nursing staff, and community to promote positive patient outcomes.

    Complet physicoail assesemtn utilizing a systems approach.

    Occasional prolong periods of sitting/driving

    Works in a heated, lighted ventilated area.

    Occasional moving of office/paper supplies up to 40lbs.

    The Volunteer coordinator directly supervises volunteers

    The Volunteer coordinator is responsible for modeling, integrity, excellence, teamwork, and innovation

    She/He is responsible for interviewing, hiring, training, and retaining volunteers; planning, assigning, and directing volunteer workload; rewarding and discipling volunteers; and addressing complaints and resolving problems.

    Particiaptes as a member of the Interdisciplinary team, including the development and implementation of the plan of care.

    Maintains accurate record of volunteer utilization as a percentage of overall patient care hours and mandated by regulations.

    Maintains records of all recruitment and retention activities.
