POSITION DESCRIPTION
OUTREACH
This is a highly visible position having a great deal of patient and staff contact and interaction. This position is responsible for outreach to the general population providing information about the Center and all services provided to potential patients, health and human service organizations in the community, safety net clinics and other non-profit organizations.
This position will serve as a “patient navigator” liaising with our community partners, churches, schools, parents and family members, colleagues, and (referral) specialists in the community. The position is mission-driven and responsible for outreach, intake and assessment, coordination of services; this individual is responsible for maintaining a monthly performance measure reporting.
This position is a member of the Outreach and Enrollment Team and acts independently and in team-settings to provide outreach with community partners, homeless shelters, schools, and churches. The position will be the first point of contact for the patient and will help asses’ patient needs and the facilitation of integrated services to ensure quality patient care, effective patient communications, and collaboration with Center colleagues.
MEDICAID AND MARKET PLACE ENROLLMENT
This position is responsible for providing education to patients with Medicaid, in the Market Place and helping to facilitate the submission of applications as appropriate. This position leads efforts in Medicaid outreach to employees of small businesses.
This position will work closely with all practices to determine eligibility for our patients and assist them to enroll in Medicaid via Virginia CommonHelp or the Market Place via Healthcare.gov. This individual is responsible for maintaining a monthly performance measure reporting.
This position will be responsible for data entry and tracking in eClinicalWorks this position will be required to self-report performance measures monthly, e.g., patients served, patients approved for insurance, patients denied, patients pending, etc. This position works with multiple other colleagues at the Center operations, quality and the four practices.
SOCIAL MEDIA
Responsible for maintaining Center’s social media platforms Facebook, Instagram up to date, including information related to events and community partners meeting. Write, develop, and strategize online content and scheduling. Will also, create and implement social media marketing adds to enhance Center’s image.
NOVA SCRIPT CENTRAL
NOVA Script Central provides quality integrated pharmaceutical care and medication access to the low-income uninsured patients. This position will focus on providing application process and follow up for individuals needing medicine for any medical condition.
SPECIALTY REFERRAL
Facilitate the coordination of patient care from their primary care provider to an outside specialist. Works with a group of specialists in the community that will see all patients including insured and un-insured. Within a community health center setting, the patient population varies greatly. It is important that the Coordinator is always able to work in a culturally competent manner. This person collaborates with the Physicians, Nurse Practitioners, and Medical Assistants to complete his/her work.
MEDICAL RECORDS
Coordinator must know and apply the policies and procedures established by the Center to ensure proper dissemination, release, scanning, and documenting of all medical records, and insurance verification tasks including the collection of any payments due. This person is the link between the Center and those inquiring of medical records.
Responsible for all incoming and outgoing medical records requests and inquiries including, but not limited to: Subpoenas, incoming ER discharge medical records or discharge summaries, medical records from patients transitioning care, and chart audits requests as applicable. Close all medical records encounters by the end of each month. Maintain a HIPAA protected environment, track, and provide as requested a log recording incoming subpoenas, payments, and outgoing medical records.
ESSENTIAL FUNCTIONS:
Educates patients and their families in the roles/responsibilities of the Center, Outreach and Enrollment Coordinator, patient/family, and state and federal government in Medicaid, Children’s Health Insurance Program. Works with patients to develop an individualized assessment and care plan target patient population (homeless, at youth risk, HIV).
Supports the planning, development, and implementation of outreach/marketing plans, e.g., health fairs, school events, shelters, etc. Provides structured patient education on health coverage and engages in follow-up conversations and offers renewal assistance for enrolled individuals.
Working with external resources, gain knowledge and maintains knowledge about eligibility and enrollment processes of public programs such as Medicaid, FAMIS and FAMIS Plus.
Respects patients by recognizing their rights and responsibilities; maintaining strict confidentiality of personal health information (PHI) and maintains quality service by establishing and enforcing organizational standards. Maintains patient care information in electronic records systems by entering new information as it becomes available; and ensuring data integrity and confidentiality.
Attends and successfully complete all required training programs. Maintains professional and technical knowledge by attending educational conferences, reviewing professional publications; establishing personal networks; participating in partner collaborations, and benchmarking state-of-the-art practices.
Assists and participates with outreach activities, local health fairs, and school fairs, special events by preparing and coordinating set up and marketing material.
Process internal and external patient referrals on behalf of the medical, pediatric, OB-GYN, dental, and behavioral health practices.
Ensure that all patient services have been pre-authorized through insurance verification.
Provide patients with information regarding health care services available in the community, including charity care.
Maintain and expand the specialist referral network, including meeting with specialists in the community willing to sign our Memorandum of Agreement.
Ensure all medical records requests and inquires are handled in a timely manner while following policies and procedures established by the Center as well as HIPPA guidelines.
Performs other duties and responsibilities as assigned by the Community Resource Supervisor.
QUALIFICATIONS, EDUCATION, AND EXPERIENCE
Candidate is required to work some late evenings and weekend hours.
Excellent written and verbal communication; Spanish language skills a plus.
3-5 years of experience in a professional medical/dental office.
