    CASE MANAGER - YOUNG ADULT THERAPEUTIC CARE

    PART-TIME / SCHEDULE TBD.

    We're looking for a PEER MENTOR FOR OUR YOUNG ADULT THERAPEUTIC CARE PROGRAM in Chicopee, MA, provide direct support on a Part-Time basis for our youth and families participating in the INTENSIVE COMMUNITY SERVICES (ICS) PROGRAM, which provides clinically intensive treatment and outreach support to help build, strengthen, and maintain youths' connections to family, home and community. The Case Manager would act as Transition Facilitator to assist young adults (YA) with mental health and behavioral health challenges to develop skills allowing them to live independently while maintaining mental health and behavioral health stability. The Case Manager would provide services in staffed or supported apartment setting, create safety, housing, employment and/or educational plans with Young Adults and coordinate appropriate services/supports as needed.  

    WHAT YOU'LL DO:

    • Provides services a manner that is strengths based, family driven, youth guided and culturally
    • Works with the Program Director, Clinician and other members of the treatment team to ensure overall professional services are delivered to
    • Solicit/receive referrals, engage and place YAs in a timely
    • Work with the program clinician to complete substance abuse, educational, job readiness assessments with participants and develop treatment plans with Young Adults to achieve
    • Provide in-home individualized case management, advocacy, safety planning and planning for housing, employment, and/or education to Young Adults, including, but not limited to, the following activities: link Young Adults to community resources to ensure mental and behavioral health needs are being managed, assist Young Adults in process of obtaining housing, working with landlords, and monitor progress with tools provided and adjust as needed.
    • Teach skills necessary, such as transportation, budgeting, that enable independent functioning.
    • Attends applicable program meetings and supervisions as well as meetings with network
    • Participates in a rotating on-call to assist and safety individuals looking for
    • Completes all required reports and tracking tools for Young Adults, funding sources and the
    • Perform other tasks as determined by the YATC Program Director, Clinician or other Department/Agency/DMH
    • Informs the Agency of all emergencies or critical incidents.
    • Performs other duties as assigned by the Program
    • Will spend significant time away from office (visiting families, collaterals, etc.).

    ADMINISTRATIVE RESPONSIBILITIES/RECORD KEEPING

    • Requirement to keep current on all Agency, Government and Funding trainings and health requirements (OSHA, Sexual Harassment, )
    • Completes all the necessary paperwork in a strength-based manner per Agency/Program
    • Attends all training and meetings as requires by the funder of the
    • Completes time sheets, mileage forms and submits them accurately and in a timely manner.

     WORKING CONDITIONS

    • This position is a 40 hour a week position. Shift hours may vary. Additionally, he/she may be required to work some holidays, nights, and/or some weekends. Office space is limited. Working conditions also include travel to and from the program and scheduled activities, appointments, client's homes This job entails high risk exposure to at risk population in high crime neighborhoods. This position involves being exposed to smoking environments.

       

    Required Skills

    WHAT WE'RE LOOKING FOR:

    • Bachelor's degree in a human service
    • It is preferred individual has at least six (6) months experience: working with the target population; or navigating any of the child and family serving systems
    • Willingness to be on-call as scheduled and work shifts if
    • Must have vehicle transportation, proof of valid driver's license, a good driving record, and vehicle
    • Must have an acceptable CORI background
    • Demonstrated ability to be innovative, creative, analytical and decisive in problem
    • Demonstrated communications and administrative
    • Ability to work effectively with diverse populations and community agencies: School Dept., DYS, Juvenile Court,
    • Understanding/willingness to be part of a Program that supports youth and families 24 hour per day, 365 days a
    • Bi-Lingual Spanish/English
