Position Overview:

Provide comprehensive medical care to residents, ensuring regulatory compliance and adherence to best practices. Manage assigned caseloads, collaborate with interdisciplinary teams, and promote person-centered care.

Qualifications:

• Education: MD or DO
• Licensure:
  • Active NYS Medical License
  • Board Certification (Internal Medicine/Family Medicine or relevant specialty)
• Experience: Experience in long-term care, geriatrics, or similar setting preferred.
• Soft Skills: Ability to work collaboratively in a multidisciplinary environment.
• Other requirements: 
  • Credentialing with MCH and payers required.
  • Compliance with HIPAA and facility policies.
  • Complete documentation per CMS and facility requirements.:

Key Responsibilities:

• Maintain licensure and board certification in a relevant specialty.
• Provide ongoing care for assigned residents, including physical examinations, care planning, and medication management.
• Supervise and collaborate with Advanced Practice Providers (APPs).
• Ensure appropriate documentation in EMR and timely completion of progress notes.
• Participate in interdisciplinary team meetings and family conferences.
• Arrange specialist consultations as necessary.
• Provide on-call and emergency coverage per Medical Director assignments.
• Ensure compliance with facility policies and federal/state regulations.
