Job Description
PRIMARY PURPOSE: Provide intensive in-home treatment and support services to families in which there is a risk for out-of-home placement of children and families in which a child is returning from residential treatment. Implement individualized, strengths-based, culturally competent clinical and support services including on-going assessment, individual and family therapy, treatment planning, case management, skill development, behavior management and system coordination. Utilize a wraparound approach, link clients with formal and informal resources and coordinate services
PURPOSE: UNDER SUPERVISION, PERFORM CLINICAL WORK INVOLVING THE DELIVERY OF MENTAL HEALTH SERVICES TO INDIVIDUALS IN NEED OF COUNSELING SERVICES. PERFORM SKILLS BUILDING, BEHAVIOR MODIFICATION, PARENTING, COUNSELING SERVICES, CRISIS INTERVENTION, AND/OR CASE MANAGEMENT SERVICES IN A HIGH-INTENSITY CLINICAL ENVIRONMENT.
DUTIES AND RESPONSIBILITIES:
- Provide case management, skill-building techniques, family and individual training, to consumers and families as appropriate to the position; prepare treatment plans, discharge plans, and follow-up care plans; provides therapeutic crisis intervention and emergency services as required.
- Gather information about consumers through interviews, collateral contact, psychological tests, and/or observational techniques; evaluates data to identify causes of problems and to determine proper therapeutic approach.
- Provide case management of a specialized caseload of clients, as appropriate to the requirements of the individual position; performs in-home visits, in-school visits; provide Behavioral Modification, Parenting, Life Skills, Anger Management, and other services as identified by Individual Service Plans.
- Maintain accurate and timely documentation of case records; modify ISP as needed; and complete other required paperwork such as monthly progress reports, etc. as needed.
- Provide, arrange, and/or refer for therapeutic interventions as appropriate for patients or clients in a crisis condition and for those with serious mental health problems.
- Follows up to determine progress and attainment of treatment goals; makes recommendations regarding modification to services and service delivery.
- Monitor medication compliance.
- Consults with other therapists and related professional and paraprofessional staff, as appropriate, in the performance of therapeutic and/or casework; refer clients to appropriate service agencies as required.
- Participates in the review of clinical issues and program policies and procedures.
- Participates in weekly Clinical Team Meetings.
- Maintain professional relationship with community agencies and referral resources.
- Performs miscellaneous job-related duties as assigned.
MINIMUM JOB REQUIREMENTS:
- Bachelor’s Degree in Counseling, Social Work, or related Field; with at least 2 year of professional experience in the field of mental health.
- Or High School Diploma with 3 years of experience with children with SED.
KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED:
- Knowledge and understanding of clinical social work and mental health counseling disorders and treatment methods, procedures, and standards.
- Good oral and written communication skills and group facilitation skills.
- Ability to work independently and meet service objectives efficiently.
- Knowledge of providing intensive case management to children with emotional disturbances and their families.
- Knowledge of proper mental health record keeping and documentation.
- Knowledge of crisis intervention techniques.
- Knowledge of community mental health resources.
TYPICAL WORKING CONDITIONS:
Most work with this agency is performed with children (under the age of 18 y/o) diagnosed with mental illness and/or substance abuse. Services are usually provided in the home, school or in other community settings. Evening and Weekend work may be required, depending on the needs of your families. The job involves collaborating with Psychologist, Psychiatrist, DFCS, DJJ, and other community agencies.
