  • Responsibilities

    JOB SUMMARY:  The MAT Coordinator is responsible for the overall care coordination, for chemical dependency, medical, mental health, and medication management for patients who are referred or identified as candidates for the MAT program. This position   requires interacting with several different community partners, and outpatient treatment services, Assures adherence to state statutes and rules, CARF standards, and federal regulations including HIPAA and 42 CFR Part 2. Positively represents the organization and promotes Rimrock’s Mission to the community.

     

    ESSENTIAL FUNCTIONS:

    1. Identify and engage MAT care coordination for patients referred to the Rimrock MAT program.
    2. Assist new patients in completing paperwork, releases of information, and all medical clearance required for program initiation.
    3. Assist MAT participants to navigate accessibility to obtain Medicaid/insurances, guaranteeing appropriate medical and mental care coverage
    4. Act as the system navigator and point of contact for patients and families.
    5. Act as a liaison between Rimrock healthcare providers and community agencies and healthcare providers.
    6. Follow and verify engagement in identified services and provide support as needed.
    7. Participate as a member of a multi-disciplinary team including but not limited to both medical and psychiatric members towards the development and management of operations of the MAT program.
    8. Assist with reporting of all safety and quality initiatives specific to the MAT program.
    9. Attend in-service trainings and related conferences and provide feedback to staff on the content of educational programs attended.
    10. Advocate on the patient’s behalf to ensure approval of the necessary supplies/services for the patient in a timely fashion.
    11. Seek opportunities to improve outcomes through population health management.

    Required Skills Required Experience

    Graduated from an accredited nursing program. Experience with the psychiatric population or substance use disorder preferred. CPR certified. 

     

  • Qualifications
    • PhD. degree in Neuroscience or a related field required
    • Minimum of 4-6 years experience in TBI research required
    • Knowledge of the principles and theoretical concepts of brain injury pathophysiology, clinical effects and treatment and general research knowledge required.
    • Ability to develop and implement scientifically sound research protocols required.
    • Must be able to effectively author professional, peer reviewed journal articles, manuscripts, research grant applications, posters, and research reports
    • Ability to select and apply statistical techniques appropriate to the brain injury research protocols) under development and/or application is required.
    • Knowledge of the administrative processes appropriate to the above assignments is required.
    • Must have excellent inter-personal and communication skills, able to develop and interact with network of researchers for sharing new research developments, identify opportunities and generate new ideas
    • Prior experience within the DoD/VA systems of care strongly preferred.
    • Must be a US Citizen
    • Must be available to travel locally, regionally and nationally.
