Geriatric Care Social Worker (MSW/BSW)

Job Overview:

Synergy Health Staffing is seeking a compassionate and experienced Geriatric Care Social Worker to support older adult patients in a healthcare or long-term care setting. This role focuses on psychosocial support, discharge planning, care coordination, and helping patients and families navigate the challenges of aging.

This is a contract-to-permanent placement opportunity with strong potential for long-term employment.

Responsibilities:

Conduct psychosocial assessments for geriatric patients

Support discharge planning and care transitions

Assist families with long-term care placement decisions

Provide emotional support and crisis intervention

Coordinate services with hospitals, SNFs, hospice, and community agencies

Support patients with dementia, chronic illness, and end-of-life care needs

Maintain accurate documentation in compliance with facility and state standards

Collaborate with interdisciplinary healthcare teams

California State Requirements:

Candidates must meet requirements regulated by the California Board of Behavioral Sciences:

Bachelor’s degree (BSW or related field) required; MSW preferred

Experience in geriatric care, healthcare, or long-term care settings

ASW or LCSW preferred for clinical roles

Knowledge of California healthcare and aging service systems

Understanding of mandated reporting requirements for elder abuse

Preferred Experience

2-3+ years in hospital, SNF, hospice, or geriatric care settings

Experience with dementia/Alzheimer’s populations

Strong discharge planning and case coordination skills

Strong communication and family support skills

Compliance Requirements

Live Scan background check (DOJ/FBI clearance)

TB test and health clearance (as required by facility)

Mandated Reporter Training (California elder abuse reporting)

Valid identification and work authorization
