Position Duties Include:
- Provide medical leadership for inpatient and SNF geriatric consultative services, to include:
- Inpatient and SNF consultations, follow-up encounters as appropriate
- This role includes leadership responsibilities for maintaining a working relationship with SNF facilities, collaborating with SNF and hospital leaders on needs, and focusing on key metrics which include but are not limited to hospital LOS, readmissions, opportunity days, other quality metrics, etc.
- Drive strategy and growth into other facilities as deemed necessary
- Family meetings as required
- Completion of capacity assessments and legal documentation as required
- Participation in multidisciplinary rounds
- Participation in clinical ethics committee
- Participation in “length of stay rounds” with nursing managers and case management, providing medical expertise for complex care management
- Participate in and lead team huddle every weekday morning as needed
- Collaboration with consulting teams, specialists, primary care physicians, case management, nursing, rehab physicians and hospice teams
- 1.0 FTE physician must provide 36 patient contact hours, 4 hours of admin time, schedule will be set 1 month in advance
- For Munson recognized holidays, if they fall on a weekday, the holiday will be covered per schedule, an alternate off day will be granted to make up for holiday coverage.
- Provide appropriate sign out to team members when going off duty, so to ensure continuity of care
- Collaborate with the Chief Medical Officer, Executive Medical Director of Medical Specialties, and Service Line Executive Director to support the needs of the hospital and health system
- Provide medical leadership to the geriatric team and leadership to help create and meet operational quality and growth goals and routine measures
- Maintain collaborative agreement with associated APPs and provide OPPE/chart review as needed
- Provide community and multidisciplinary education regarding geriatric medicine including end of life decision making, management of geriatric syndromes and symptom management emphasizing pain control
- Provide education and direction to medical students and residents that may be on service
- Participate in regional/community events in collaboration with public relations department including participation in regional committees that would facilitate the dissemination of education throughout the community
- Provide geriatric medicine education to primary care and inpatient providers and serve as a resource for these providers for questions regarding specific care plan
- Participate in monthly geriatric practice council meetings
- Provide on call services for patients as is reasonably requested by the HOSPITAL (currently no after hours nor weekend coverage required).

- Maintain a working knowledge of applicable Federal, State and local laws and regulations, HOSPITALS corporate compliance Program, Code of Ethics, as well as other policies and procedures to ensure adherence in a manner that reflects honest, ethical and professional behavior
- Maintain CME, board certification, licensure and credentialing through the medical staff
- Participate in quality improvement activities as appropriate and as assigned and report results on a routine basis to appropriate medical quality improvement programs
- Participate with planning and implementing operational decisions, policies and procedures relative to geriatric medicine
- Cooperate with HOSPITAL in marketing geriatric medicine services
- Provide input regarding equipment needs, providers and standards required and other appropriate process ease, and communicate these to appropriate administrative staff
- Actively work with HOSPITAL to ensure patient flow and access, monitoring and controlling cost, in utilization of services
- Participate with management, planning and implementing programs to enhance services
